Please send all the required documents with your check or money order to BCV (FedEx, UPS, or
USPS Express recommended). Multiple applications can be sent and returned in one package.

Speical Requirements For Minor Under 18:
  • For VISA applicants under 18 years of age, both parents must sign the visa application form and have
    their signatures notarized. In addition to the items mentioned above, the following documents should
    also be brought or sent to the Consulate:
  • A notarized copy of the minor birth certificate;
  • A notarized travel authorization form (two pages); this is mandatory, even if both parents are traveling with
